How do you avoid mistakes in the lettering?

Engraving cannot be corrected once it has been cut, so treat the wording like something official. Write it out in full, capitals included, and check the name against something written, such as a vet record or a collar tag. Ask someone else to read it back before you order. If you are unsure about including a date, leave it out: a name alone is never wrong.
